Registering as an NHS patient

Arduthie NHS Dental Practice, Stonehaven, AberdeenshireThere are certain restrictions to treatment available under the NHS although your dentist can provide you with treatments not covered by the NHS under private terms. Please ask for further details.

You do not have to pay for your NHS treatment if you are:
•  Under 18
•  Age 18 and in full time education
•  Expecting a baby
•  Had a baby in last 12 months
•  Named on a HC2 certificate
•  You or your partner is in receipt of:
____• Income Support
____• Income-based Job Seekers Allowance
____• Pension Credit Guarantee Credit
____• Tax credit /hold a valid tax credit exemption certificate
____• Income related employment support allowance

If you do not fall into any of the categories but are on a low income you can complete an HC1 form which enables you to apply for help towards dental costs. This form is available from main post offices and can be available at GP surgeries. Click here for more information.

Please bring proof of exemption with you to your first visit. This will ensure you will not have to pay any charges.

You can pop into the practice to sign a form to register or if you are registering a child, registration can only take place when an examination has been carried out by the dentist. If you are exempt from dental charges, please let the reception staff know and remember to bring your proof of exemption with you.

We will ask you to complete a medical questionnaire which helps us to ensure you receive the appropriate care and advice in relation to your medical status.

Should you be unable to attend appointments, we ask that you give 24 hours notice or a charge may apply if we are unable to fill the appointment.
